CS-011: MVA Criminal Record Request Form

Form should be used to process a Maryland criminal background check. The form must accompany all applications from licensees or anyone with a financial interest in a business. Must include a photocopy of a Maryland Photo Driver’s License or a Maryland Photo ID, current address, and original signature. The completed form must be submitted in person at a full-service Maryland Motor Vehicle Administration office.

 

VR-005:Application for Certificate of Title

Form should be used for titling a vehicle. The completed form must be submitted in person at a full-serviceMaryland Motor Vehicle Administration office.

 

VR–O08: Application for New Plates/Stickers & Transfer of Plates or Non-Title Trailers

Form should be used to apply for new license plates and registration stickers or to transfer license plates or non-title trailers. The completed form can be brought in person to a local full-service MVA office or mailed along with the appropriate fees to: Maryland Motor Vehicle Administration, 6601 Ritchie Hwy., Glen Burnie, MD 21062, Attn: Vehicle Registration.

 

VR-009: Application for Substitute Plates/Stickers/Duplicate Registration Card

Form should be used to apply for substitute license plates, registration stickers or a duplicate registration card. The completed form can be brought in person to a local full-service MVA office or mailed along with the appropriate fees to:Maryland Motor Vehicle Administration, 6601 Ritchie Hwy, Attn. Registration Renewal Section, Glen Burnie, MD 21062.

 

VR 018: Application for Duplicate Certificate of Title

Form should be used to apply for a duplicate title for a vehicle. The completed form can be brought in person to a local full-service MVA office ormailed to:Maryland Motor Vehicle Administration, 6601 Ritchie Highway, N.E., Glen Burnie, MD 21062, Attn: Title File, Room 104.

 

VR-021: Application for Registration Plate Refund

Form should be used to apply for a refund for registration plates. The completed form can be brought in person to a full-service MVA or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Hwy., NE, Glen Burnie, MD 21062, Attn: MVA Tag Return.

 

VR-096: Application for Historic or Street Rod Registration

Form should be used to apply for historic or street rod license plates. All necessary documentation must be provided. The completed form can be brought in person to a local full-service MVA office or mailed along with the appropriate fees to:Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, Maryland 21062, Attn. Vehicle Registration.

 

VR-103: Application for Maryland Gift Certification

Form should be used for titling a vehicle if it is to be given as a gift from one family member to another family member. The completed form must be brought in person to a local full-service MVA office.If transferring a vehicle from an Aunt or Uncle to a Niece and/or Nephew you will need to have the Certified Statement form (form # VR-299) accompany the Gift Certification form.

 

VR-129: Temporary Inspection Waiver

Form should be used to waive a temporary inspection of a vehicle. The completed form can be brought in person to a full-service MVA or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 30, Glen Burnie, MD 21062.

 

VR-154: Application for Maryland Change of Address and/or Name

Form should be used to register to vote, to change an address or change a name. The completed form can be brought in person to afull-service MVA office or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Room 203, Glen Burnie, MD 21062. Refer to the Maryland State Board of Elections for information about Maryland Elections.

 

VR-164: Application for Special Registration Plates

Form should be used to apply for personalized (vanity) license plates or license plates for amateur radio operator registration. The completed form can be mailed along with the appropriate fees to: Maryland Motor Vehicle Administration, 6601 Ritchie Highway, Glen Burnie, MD 21062, Attn: Vehicle Registration.

 

VR-210: Application for Maryland Parking Placards/License Plates for Individuals with a Disability

Form should be used for individuals with a disability to apply for a parking placard or license plate. Your physician will need to complete the disability certification portion of the application. The completed form can be brought in person to a full-service MVA office or mailed to: Maryland Motor Vehicle Administration, Disability Unit, 6601 Ritchie Highway, Glen Burnie, MD 21062.

 

VR-217: Security Interest Filing Statement

Form should be used to record a lien on an existing title certificate or for the original lien holder to reassign the security interest in a vehicle. The Certificate of Title must accompany this form. The completed form and title, along with the appropriate fees, can be brought in person to afull-service MVA office or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Hwy N.E., Glen Burnie, Md. 21062, Attn: Title File, Room 202.

 

VR-302: Application for Chesapeake Bay/Agricultural License Plates

Form should be used to apply for the Commemorative Chesapeake Bay License Plate or the Agricultural License Plate. The application includes background information about the Chesapeake Bay, farming and agriculture in Maryland. The completed form can be brought in person to a local full-service MVA office or mailed to: Maryland Motor Vehicle Administration, 6601 Ritchie Highway, N.E., Glen Burnie, MD 21062.
